Dr. David Canfield received his dental degree from UCLA and completed a Comprehensive Residency in Anesthesiology at the
University of Pittsburgh. He is a fellow in General Anesthesia, American Dental Society of Anesthesiology. He is
an anesthesia consultant for the Texas Department of Mental Health & Mental Retardation and The VA Medical Center. He
maintains a private anesthesiology practice in the D/FW area. Dr. Clark Whitmire maintains
a private anesthesiology practice in Houston and is an Assistant Professor of Anesthesiology in the Department of Oral and
Maxillofacial Surgery at UTHSC Dental Branch. He earned his DMD degree from the University of Alabama and completed a Comprehensive
Residency in Anesthesiology at the UMDNJ-Robert Wood Johnson Medical School. He is a Fellow in the College of Legal Medicine
and a Fellow in the American College of Dentists.

Enteral Sedation Permit Renewal Course Description:
This continuing education program satisfies Rule 110 by the Texas State Board of Dental
Examiners, and other dental state boards, requiring that all practitioners, in order to keep their Enteral Sedation permit
current, document successful completion of a continuing education course every three years. |
Enteral 2-day Initial Permit Course Description: This
course will provide the necessary training to satisfy the requirements to obtain an Enteral Conscious Sedation permit. Designed
to provide an excellent training and review of pretreatment evaluation and management of office emergencies, this program
will benefit all healthcare providers (this course is for practitioners seeking to obtain an Enteral Sedation permit).
